Craftsman model 5120 ½hp 3450rpm 7.3A bench grinder (catalog #2609). This model would later be known as 115.6332.
Type P_C, Manufacturers code K_4_8. Wheels are 7" x 1" face and 5/8" shaft. Ball bearing and capacitor start.
Cast iron base, end bells, guards and tool rests. The tool rest adjustment bar is solid steel which passes through the width of the base.
Interesting feature: adjustable spark shields are located in the exhaust ports of the wheel guards.
Missing: eye shields LH tool rest (RH currently shown on left side) and water tray.
The LH guard is chopped. The RH guard and base has a chunk of iron missing.

September 2009:
I purchased this grinder from Pleasant Street Machinery, during ArnFest 2009. No further history available.

UPDATE-September 2015:
I no longer own this grinder. Sold to a fellow OWWMer, Ruckered to ArnFest 2015.
