As of December, 2008, this saw belongs to a fellow in Lebanon, TN. He told me that he worked for a furniture manufacturer in North Carolina years ago, and the saw was operated there with one specific setup. When the manufacturer no longer needed it, they scrapped the saw and he acquired it.
This saw originally had a 10hp, 3-phase motor, which the owner has replaced with a 5hp, 3-phase. He has built his own phase converter to operate it! He does not tilt the table, because the mechanism isn't exactly sophisticated or easy to adjust; he has another, more modern table saw that he uses for angle cuts.
Sorry for the picture quality; these were made with my cellphone camera. As far as I know, this saw is not for sale.
