The original bearings a "New Departure" 05, a single row, felt shielded bearing made in US.

Thanks to Jim L aka 63pro I have some information on the replacement bearings:

Replacements are available from Accurate Bearing Co., talk with Lynne Scott who can be reached on a toll free line at 800-323-6548. Both bearings in the headstock will be replaced by a bearing numbered 87505 aka 205KLD.

Be sure to mention the Old Woodworking Machines.
