Sears Dunlap/Central Specialty 4"x36" belt/8" disk sander, model 103.08011, catalog no. 2213.
All bronze bearings feel good.
By design, only the idler roller has the rubber sleeve.
It appears to be missing the following: one bearing oil plug and the belt stop/fence.

I bought this sander from a Craigslist seller in Warren Michigan, on 23Oct2006.
Component casting numbers.

UPDATE-July 2007:
Found a belt fence (592-6) at a resale shop.
Used clear RTV to glue rubber sleeve to idler drum--runs and tracks properly now.
UPDATE-January 2013:
Attached sander to a 11x26 board.
The sander is powered by a Delco 1/3 hp motor with 2" pulley and ½x44 V-belt.
A Chicago Die Casting motor arbor was added to the open sander drum shaft end, to turn small accessories.
Attempted to run a second sanding disk on the open shaft end but machine vibration became excessive.
UPDATE-July 2018:
Attached sander to a Craftsman 16x30 Universal Stand.
The sander is currently powered by a Craftsman/Leland 1/2 hp 1725 rpm RI Oilite bearing motor.
A Chicago Die-Casting 5/8 bore motor arbor was added to the drive roller shaft end with a sanding flap wheel.

The table can be moved to the belt or disk. The disk can be used on either side.
