This was for sale at a recent industrial auction in my area, and unfortunately I did not end up as the high bidder. However I was able to capture some photos. Since we have very few examples of this setup I've included a set here to give us another example.
One things I note is that this table has a square opening for the insert plate while the other example of this setup we have in the photo index had a round insert. This table had a Unisaw fence and rails attached, and a box built for around the router to contain the sound.
