This is a 3/16" to 3/4" chuck made by T. R. Almond that an OWWM member was kind enough to sell me. It is stamped "ALMOND CO - ASHBURNHAM MASS - MOD 40-101". A Jacobs #4 key fits it perfectly. This doesn't have a Jacobs taper but instead has a 1-7/64" straight wall hole for an arbor. It has two 5/16" set screws to secure it to an arbor. I cleaned it up and oiled it and it runs very smooth. I don't know how accurate it runs yet, but I will machine a MT4 arbor for it and use it on my larger lathe.
If anyone can help me date it, I'd appreciate it.
