I've posted this Craftsman Power-Panl receptacle box for historical record purposes.
I obtained this Power-Panl along with a Craftsman/King-Seeley table saw 103.22160, 1/2HP motor and 16"x30" power tool bench from a Craigslist ad.

The toggle switch is rated for a 1 HP motor.
Two threaded studs with nuts, on the top edge, attach to the power tool bench.
The back has the following:
- one two prong male 120V connector for a power cord.
- one powered two slot female 120V outlet, on the right side, for a work light.
- one switched two slot female 120V outlet, on the left side, for the motor plug.
