This came from a local listing, offered by a seller who pulled it from his deceased father-in-law's shop some years ago. No known history is available prior to that. It is in plug-n-play condition, as the seller has used it a few times on some projects in his own shop. Given the rarity of the machine, and the maker in general, I couldn't walk away from it even though I originally went just to take some photos for these archives. It's now stored at my place.
EDIT: A couple months after arrival, the machine restoration has been finished. I replaced the original 1/4hp split phase Apex motor with a 1/3hp Jack & Heintz (JAHCO) motor that I had in storage for a few years. To fit the 5/8" motor shaft, I found a suitable ShopSmith drill chuck that direct mounts to the straight shaft with a locking set screw.
