Found on local Craigslist, from the garage of a seller who had brought it home several years ago from work when OSHA kicked it out of his employer's shop due to no guards. Plug and play as it is shown in the photos below, down to a compatible plug with my 3-phase setup. I have no idea when it may have been manufactured, will try to remember to update this listing if I find out.
