This motor came mounted to an early Delta model 30-C Radial Arm saw. The saw dates to 1951 so I'm guessing the motor did as well. Though AO Smith is reported as located in Milwaukee, WI this motor is tagged as being from Tipp City, Ohio, U.S.A.

PO must have tried to protect wiring diagram with shellac. Due to it yellowing I am unable to decipher it completely due to the yellowing.

Some of these motors came equipped with a toggle switch to change voltage without the need to switch wires around inside motor. This motor obviously did not come with a switch.
