Found this at an estate sale for $30 including 2 other chucks. It is not on the original base plate and I cannot locate a serial number or any other identifiers on it. Everything works and only surface rust. Looking forward to cleaning it up. Hopefully I can find some documentation on it.

[Edit. Jul 10, 2016] New pictures after cleaning it up. Disassembled everything but the spindle. Ran all the bare metal pieces through a wire wheel, and put a coat of paste wax on it. Replaced the exposed wiring toggle with a switch and box.
